This is your club registration and parental consent form, for the below named

member for season 2021/22.


Membership Fee Payment

The one-off membership fee of £10 (junior players) or £20 (Mens team) can be paid in the following ways, cash or bank transfer. Bank details and the player's personal membership number will be provided seperately. Cash can be passed directly to the team manager / coach.

Please note this membership fee is non-refundable should you choose to leave the club during the season.


Membership Fee Payment

The 12 monthly subs fees (as detailed) below are to be paid directly to the bank details that will be provided seperately. Please ensure when transferring funds or setting up any Standing Orders that you include the player's unique membership number in the reference field on your online banking.


Single player - £20 per month

Single player plus sibling - £30 per month

Single player playing Saturday & Sunday - £30 per month

Single player playing Saturday & Sunday plus sibling - £45 per month


The first payment will be expected by the 31st August 2021 and all other payments in the subsequent 11 months must be paid by the 10th.

There is also the option of a one-off cash or cheque payment for £220 this will save you £20 in your yearly membership.

 may be used by the press, we will gain your permission for separately.

We all have a responsibility to promote high standards of behavior in the game

This club is supporting The FA’s Respect program me to ensure football can be enjoyed in a safe,

positive environment.

Remember children’s football is a time for them to develop their technical, physical, tactical and social skills. Winning isn’t everything.

Play your part and observe The FA’s Respect Code of Conduct for spectators and parents/carers at all

times


    I will:   

• Remember that children play for FUN

• Applaud effort and good play as well as success

• Always respect the match officials’ decisions

• Remain outside the field of play and within the Designated Spectators’ Area (where provided)

• Let the coach do their job and not confuse the players by telling them what to do

• Encourage the players to respect the opposition, referee and match officials

• Avoid criticizing a player for making a mistake – mistakes are part of learning

• Never engage in, or tolerate, offensive, insulting, or abusive language or behaviour.

I understand that if I do not follow the Code, any/all of the following actions may be taken by my

club, County FA, league or The FA:

    I may be:   

• Issued with a verbal warning from a club or league official

• Required to meet with the club, league or County FA Welfare Officer

• Required to meet with the club committee

• Obliged to undertake an FA education course

• Obliged to leave the match venue by the club

• Requested by the club not to attend future games

• Suspended or have my club membership removed

• Required to leave the club along with any dependents.

As a player, you have a big part to play. That’s why The FA is asking every player to follow a Respect Code of Conduct.


When playing football, I   will:    

• Always play to the best of my ability

• Play fairly – I won’t cheat, complain or waste time

• Respect my team-mates, the other team, the referee or my coach/manager

• Play by the rules, as directed by the referee

• Shake hands with the other team and referee at the end of the game

• Listen and respond to what my coach/ team manager tells me

• Talk to someone I trust or the club welfare officer if I’m unhappy about anything at my club.


I understand that if I do not follow the Code, any/all of the following actions may be taken by my club, County FA or The FA:


  I may:    

• Be required to apologise to my team-mates, the other team, referee or team manager

• Receive a formal warning from the coach or the club committee

• Be dropped or substituted

• Be suspended from training

• Be required to leave the club

In addition:

• My club, County FA or The FA may make my parent or carer aware of any infringements of the Code of Conduct

• The FA/County FA could impose a fine and suspension against my club.




To comply with new GDPR regulations, your information will only be shared with your child’s coach and assistants for the purpose of organising the squad, keeping your child safe and maintaining an emergency point of contact. Completed forms will be kept in a locked filing box at the home of the Welfare Officer and destroyed at the end of each season.


I consent to the above details being maintained in a computerised record held by Allenton United Football Club, Derbyshire County FA and the League which my son/daughter is playing within. I understand that mine and my child’s details will be held for registration, administration and statistical purposes.


I understand that the consented information will be made available to Allenton United Football Club committee members, coaching staff or medical authorities only, and used solely in connection with Allenton United Football Club matters.


I also consent to my child receiving medication and any emergency dental, medical or surgical treatment, including anaesthetic or blood transfusion, as considered necessary by the medical authorities present.


I wish for my son/daughter to be registered with Allenton United Football Club for the 2021 / 2022  season. On signing this form, I certify that my child has not signed a registration form for another team within another club that competes in the same league as Allenton United Football Club in the current season.


I certify that I have discharged all financial liabilities to my child’s previous club(s). I certify that my child is of the relevant age to compete in the above age group.


  I understand and accept if no payment is made by the 31st of August 2021 my membership will be suspended until full payment is made. And that my son/daughter won’t be eligible for match selection or permitting to train during this time.

  Any player who isn’t fully up to date with subs payment will not be permitted to attend the end of season club presentation.
